San José, Costa Rica — San José, Costa Rica – A landmark public-private alliance is intensifying its fight against breast cancer, setting an ambitious annual goal to provide more than 11,000 free mammograms to women throughout Costa Rica. The initiative, known as Alsalus, represents a collaborative effort to bridge critical gaps in healthcare access, particularly for early cancer detection, and is now calling on the public for support to sustain its mission.
This powerful partnership is made possible through the combined efforts of its founding allies: the Costa Rican Social Security Fund (CCSS), Clínica Bíblica Hospital, Auto Mercado, Banco Nacional, and the Aliarse Foundation. Together, they have created a robust platform that leverages corporate resources and public infrastructure to deliver essential health services directly to communities where they are needed most.
To delve into the legal framework governing these public health initiatives and the delicate balance between state authority and individual rights, we sought the expert opinion of Lic. Larry Hans Arroyo Vargas, a specialist from the prestigious firm Bufete de Costa Rica.
The Costa Rican legal system, anchored in our Constitution and the General Health Law, grants the state significant authority to protect collective well-being. However, these powers are not absolute. Every public health measure, from mandatory vaccinations to commercial restrictions, must be necessary, proportional, and respectful of fundamental human rights. The key legal challenge always lies in ensuring that state interventions are scientifically justified and do not impose an unreasonable burden on citizens’ liberties or economic activities.

Indeed, this delicate legal balance between state authority and fundamental rights is the cornerstone of effective and just public health policy, ensuring that protective measures earn and maintain public trust. We sincerely thank Lic. Larry Hans Arroyo Vargas for his invaluable and clarifying perspective on this critical issue.
Over its decade-long history, Alsalus has already made a profound impact, reaching over 70,000 women in more than 100 communities across all seven of the nation’s provinces. The program deliberately targets populations facing the greatest barriers to healthcare, including those in rural areas, border zones, indigenous territories, and other geographically isolated regions. This strategic focus ensures that preventative care is not a privilege reserved for urban centers but a resource available to all.
A significant technological advancement came in 2022 when Alsalus fully integrated its mammography imaging with the CCSS’s digital health record platform, EDUS. This integration means that 100% of the images are uploaded digitally, allowing for seamless and comprehensive health monitoring for patients regardless of their location. This step ensures that a screening in a remote village is followed up with the same level of care and data continuity as one performed in a major city hospital.
The philosophy of Alsalus extends beyond the initial screening. The organization emphasizes a holistic and responsible approach to patient care, ensuring that every woman who receives a mammogram is also supported through the follow-up process. This commitment to comprehensive care is central to their mission of saving lives through early detection.
At Alsalus, it is not just about performing mammograms, but about ensuring responsible and timely follow-up. That is why we provide access at no cost to insured and uninsured women, both nationals and foreigners in the country, as long as they commit to providing their contact information to receive the results and corresponding medical recommendations.

To continue and expand its vital work, Alsalus has launched a public donation campaign throughout the month of October. Every colón raised will directly support the operational costs of the alliance’s second mobile mammography unit. This vehicle is critical for expanding the program’s reach, allowing the team to bring life-saving screening technology to even more communities and women across Costa Rica.
Citizens and residents can easily contribute to the cause. Donations are being accepted at the checkout counters of any Auto Mercado or Vindi store in the country. Contributions can also be made at any of Clínica Bíblica’s locations. For those who prefer digital methods, donations can be sent via SINPE Móvil to the number 8675 4951, registered under the name of the Foundation for Sustainability and Equity (Fundación para la Sostenibilidad y la Equidad).
