The renewed agreements involve four established partners: the University of Medical Sciences (UCIMED), the Ibero-American University (UNIBE), the Autonomous University of Central America (UACA), and the Hispano-American University (UH). In a notable expansion, two new institutions have joined the program: the Latin University of Costa Rica (ULatina) and the International University of the Americas (UIA). Their inclusion is set to broaden the academic offerings and create more opportunities for aspiring specialists, directly tackling potential shortages in various medical fields that could otherwise strain public health services.

The CCSS has aggressively pushed to increase capacity, announcing the creation of over 400 new training positions. Officials have confirmed their intent to offer a similar number of slots for the 2026 intake, signaling a sustained commitment to closing the specialist gap. Currently, there are 954 doctors undergoing specialist training across diverse branches of medicine within this framework.

To ensure the highest standards are met, the selection process for these coveted positions has been refined. Dr. Juan Carlos Esquivel Sánchez, the executive director of the Center for Strategic Development and Information in Health and Social Security (Cendeisss), outlined a rigorous, two-tiered evaluation system that was first implemented in 2024. All candidates must first pass a comprehensive national examination before proceeding to a second, internal test administered by each participating university.
