The findings stem from the TARGET-D randomized clinical trial, conducted by researchers at the Intermountain Medical Center in Utah. The trial enrolled 630 adults between 2017 and 2023, all of whom had previously been treated for acute coronary syndrome. An initial assessment found that a staggering 85% of these participants had insufficient levels of vitamin D, highlighting a prevalent deficiency in this high-risk population. The participants were then divided into two groups: a control group that received standard post-infarction care and a treatment group whose vitamin D3 supplements were actively managed to maintain blood levels between 40 and 80 ng/ml.
Over an average follow-up period of 4.2 years, the results were striking. The rate of a second heart attack in the group receiving personalized supplementation was just 3.8%, a significant drop from the 7.9% rate observed in the standard care group. It is crucial to note, however, that this profound benefit was specific to reducing the incidence of new heart attacks and did not show a statistically significant impact on other major adverse cardiac events, such as stroke or heart failure.
The key to the study’s success was its individualized protocol, which moved beyond one-size-fits-all dosage recommendations. This allowed researchers to safely administer the necessary amounts of vitamin D3 to achieve and maintain therapeutic blood levels for each patient.

Despite the promising results, the American Heart Association has emphasized that these findings are preliminary and have not yet undergone the rigorous peer-review process required for formal publication. The researchers also pointed to several limitations, including a lack of diversity in the study population, which was predominantly Caucasian. This makes it difficult to generalize the findings to other ethnic groups and highlights the need for broader, more inclusive research.
While this study opens an exciting new frontier in cardiac care, experts caution against self-prescribing high doses of vitamin D. The American Heart Association stresses that supplementation must be individualized and supervised by a medical professional. An excess of vitamin D can lead to serious health complications, including hypercalcemia (dangerously high calcium levels in the blood), kidney failure, and cardiac arrhythmias. Further large-scale studies are required to validate the efficacy and safety of this personalized strategy before it can be recommended as a standard of care.
