Soto argues that robust healthcare systems should not be viewed as black holes for public treasury funds, but rather as powerful engines of economic productivity and social stability.
We believe that health should be understood as an investment that must generate a good return, rather than as a budget expense. If a person receives comprehensive management of their disease, especially in complex ones, they obtain better clinical results, prolong their life, and manage to return sooner to their personal, family, and productive activities. There are studies indicating that for every dollar invested in health, between two and four dollars of economic benefit can be obtained through higher productivity and fewer losses from absenteeism, disability, or premature death. To ground this philosophy in tangible results, ImpulSALUD is launching its inaugural pilot programs in Costa Rica and the Dominican Republic, focusing directly on breast cancer. The medical and economic logic behind this choice is clear: treating breast cancer in its early stages is not only clinically superior, often offering a path to a complete cure, but it is also up to three times cheaper for public systems than managing late-stage, metastatic cases that require prolonged, invasive, and palliative therapies.
The pilot programs leverage advanced digital solutions, data analytics, and artificial intelligence to assist radiologists in reading mammograms. By streamlining the screening-to-treatment pipeline, the initiative aims to eliminate the administrative bottlenecks and long waiting lists that frequently delay life-saving care. This targeted application of technology ensures that patients are connected to the right specialist at the right time, maximizing clinical efficacy.
In Costa Rica, the program is being designed to align with and accelerate the existing goals of the Costa Rican Social Security Fund (CCSS) and the Ministry of Science, Innovation, Technology, and Telecommunications (MICITT). A key component of the strategy involves optimizing the country’s existing Digital Health Record system (EDUS). Analysts estimate that integrating these precision-focused screening and diagnostic pathways could save the CCSS upwards of $135 million over the span of a decade, while dramatically elevating patient outcomes.
Crucially, Roche emphasizes that ImpulSALUD is not designed to dictate policy to sovereign healthcare institutions, but rather to serve as a catalyst. By building alliances that bridge the public sector, academia, multilateral organizations, and private enterprise, the movement aims to co-create sustainable, scalable solutions that no single entity could achieve independently.
We are not convincing institutions that this is important; the institutions already have this very clear and have plans in this regard. Our goal is to help accelerate them and become a strategic ally that contributes knowledge, scientific evidence, and experience to transform hospital and medical services in the region through public-private collaboration. The philosophy is to recognize that no single organization possesses all the answers on its own and that it is key to join forces and share specific capabilities because countries are not prepared to face the health crisis in front of them using only public resources.

As the year 2026 draws to a close, the focus shifts to the practical implementation of these pilot programs. By demonstrating clear, data-driven successes in early breast cancer detection, ImpulSALUD intends to establish a repeatable, scalable blueprint. Ultimately, the goal is to expand this investment-heavy, preventative approach to other chronic pathologies, securing both the physical well-being of the population and the long-term fiscal viability of regional healthcare networks.
